Evelyn I. Kozub 99 of Hawkins died on Sunday, December 6th, 2015 at Ladysmith Care & Rehabilitation Center.
Evelyn is survived by 3 sons; Ray of Sparta, TN, David of Montana and Keith of River Falls; 7 grandchildren, 4 great-grandchildren and 1 great-great-granddaughter, 1 brother and 1 sister: Don Anderson of Ladysmith and Lorraine Adkins of Illinois.
A Memorial Service will be held at 11:00 AM on Saturday, December 19th, 2015 at Bethel Lutheran Church in Hawkins with Rev. Jody Becker officiating. Evelyn's family will receive friends for an hour prior to service time at the church.
