Michael H. Dvorak, 47, of Ladysmith, died on Monday, July 13th, 2015 at Sacred Heart Hospital in Eau Claire
Mike is survived by his wife Merri, 2 sons and 1 daughter: Dustin of Eau Claire, Mathew of Renton, WA, and Crystal of Shell Lake, WI, his father Art of Ladysmith, 2 brothers and 1 sister: Russ of MN, David of Eau Claire and Rosemary Downing of Braham, MN.
Memorial services will be held at 4:00 PM on Sunday, July 19th, 2015 at The Nash-Jackan Funeral Home in Ladysmith with Deacon Doug Sorenson officiating. Mike's family will receive friends from 3:00 PM until service time on Sunday at the Funeral Home.
